FMHVR: Located on the metal plate on the right side of the visor — look for "HVR" above the CCM logo.
ACCHVR: Engraved into the polycarbonate at the top-right corner of the visor — begins with "SR".
If you cannot locate the serial number, leave this field blank.

Product return

After submitting this form, you will receive a confirmation number by email, along with further instructions. You will need to bring the confirmation number and the recalled product to a participating retailer near you in order to receive your free replacement visor.
